✔️ Uses of Zepatid

• To improve glycemic control in adults with Type 2 diabetes mellitus
• Management of Obesity

✔️ Dosage & Administration of Zepatid

• Route: Subcutaneous injection only.
• Initiation Dose: 2.5 mg once weekly (not for glycemic control, only for initiation).
• After 4 Weeks: Increase to 5 mg once weekly.
• Further Adjustments: Can be raised in 2.5 mg steps after 4-week intervals if needed.
• Maximum Dose: 15 mg once weekly.

Missed Dose:

• Take within 4 days of the missed dose.
• If more than 4 days pass, skip it and resume the regular schedule.

Injection Guidelines:

• Inject into the abdomen, thigh, or upper arm; rotate sites.
• Can be taken with or without meals, any time of day.
• If used with insulin, inject separately in different spots, but same region is acceptable.

Handling Instructions:

• Do not freeze or expose to light.
• Do not use if frozen or discolored.
• Do not mix with other solutions or medications.
• The needle cap contains natural rubber (latex derivative) and may cause allergic reactions.

✔️ Interaction

• May slow stomach emptying, affecting absorption of oral drugs.
• Monitor patients using drugs with narrow therapeutic windows (e.g., warfarin).
• Dose adjustments may be needed when taken with insulin or sulfonylureas to prevent hypoglycemia.
• Women on oral contraceptives should use non-oral or barrier methods during the first 4 weeks of therapy and 4 weeks after each dose increase.

✔️ Contraindications

• Known allergy to tirzepatide or any ingredients in Zepatid.
• History (personal or family) of medullary thyroid cancer or MEN 2 syndrome.

✔️ Pregnancy & Lactation

Use only if the benefit justifies potential risks to the fetus. Consider both maternal health needs and potential risks to a nursing child when used during breastfeeding.

✔️ Precautions & Warnings

• Inform patients about possible thyroid tumors (MTC).
• Monitor for signs of pancreatitis and discontinue if suspected.
• Adjust insulin or sulfonylurea dose to lower risk of hypoglycemia.
• In renal impairment with GI symptoms, monitor kidney function closely.
• Watch for progression of diabetic retinopathy.
• Evaluate gallbladder health if symptoms of gallstones appear.
• Discontinue and treat if allergic reactions occur.
